When is a driveway permit necessary and how do I obtain one?

Driveway permits are necessary for the installation of a new driveway or for existing driveways if making any structural changes/additions/etc., such as replacing a culvert pipe.  

A driveway permit is required before being issued a building permit via the Building/Planning department or obtaining a legal address.
